Support Point Coordinators will connect you to the providers who provide practical support, advocacy, and structured programs to help individuals transition from hospital care to community life or prison release.

Building Confidence and Safety After Hospital
Hospital discharge marks a significant achievement, yet it creates intense feelings of overwhelm for many participants. NDIS participants and their families experience a time of change that involves learning new behaviour patterns, emotional readjustments, and confusion about their future steps. Support Point Coordinators coordinate and connect you with providers who can deliver person-centred Post-Hospital Discharge Support through practical steps to meet all recovery requirements.
Personalised Transition Planning Schedule
The planning process begins when hospital discharge becomes imminent. Our Support Coordinators work with hospital staff, families, GPs, and community providers to develop individualised transition strategies. This includes:
- Transport home after discharge and safe arrival.
- Medication management and clear instructions for new prescriptions.
- Home modifications, assistive technology setup, and equipment delivery.
- The service organises personal care support, which includes help with continence care, wound treatment, and food preparation.

Support Point’s team helps participants rebuild their independence through a Coordinated approach that includes medication management, meal preparation, personal hygiene, and mobility tasks.
Flexible, Responsive Service
No two recoveries are the same. Support Point’s coordination team will ensure the provider will adjust to your evolving changes in health status, family participation, or new NDIS objectives. We provide short-term, intense post-discharge care and ongoing help for people needing extensive support.
Emotional and Social Support
After hospital discharge, people may experience emotions such as anxiety, sadness, or concern about their future. Our team connect you with peer support and community organisations to rebuild your social connections and minimise feelings of loneliness while promoting lasting recovery.
Home Environment Readiness
Homecoming should bring a sense of safety along with welcoming comfort. Support Point facilitates home modifications, including ramp installation, handrail placement, and special bed setup (if you have funding available). Our coordinators will organise professionals to ensure all required equipment operates correctly before the participant returns home, thus minimising risks during the transition period.
